How To Replace Your Oregon Drivers License

If your Oregon drivers license has been lost, stolen or destroyed, you'll need to get it replaced immediately. We offer links to resources that will save you time and get you going in the right direction.

Replacing Oregon Drivers License

In order to replace your Oregon drivers license, you must:

  • Be present in person at the Oregon DMV office of the region where you reside
  • Submit completed application form
  • Submit the required fees in cash. Most DMV locations have ATM for convenience of the applicant.
  • Submit all the documents required
  • Have photograph taken

Required Documents
Oregon drivers license replacement applicants will be required to bring with them documentation. Documentation is needed to proof identity, proof residency and citizenship.   • Proof of full legal name
    • U.S. government issued birth document 
    • U.S. consular report on birth abroad 
    • Valid US passport or passport card 
    • A tribal ID card 
    • U.S. immigration documents 
    • U.S. naturalization documents 
  • Proof of legal presense in the United States; Documents acceptable are same as above.
  • Social Security Number 
  • Proof of residence address
  • U.S. passport or passport card
    • A person who lives with the applicant must accompany applicant and present proof of residence address.
    • Oregon ID card not expired for over a year 
    • Oregon vehicle registration and title showing the same address as which applicant resides now 
    • Utility statement 
    • Any document issued by a financial institute that includes residence address 
    • Any item delivereed by U.S. postal service, FedEx or UPS by verifiable business or government agency 
    • U.S. government issued marriage certificate 

Oregon Drivers License Replacement Out-Of-State
If the applicant is out of state, he or she can still replace a drivers license. Oregon drivers license replacement applicant may be eligible to apply for a “valid with previous photo (VWPP)” replacement drivers license. Applicants should check online for their eligibility for making this request. If they have it they can provide a valid email ID and phone for DMV to contact them.
